Salus Case Management Services

Salus case management services are provided with the primary goal of enabling adults with serious mental illness to find housing, employment and participate more fully in their community. Ultimately case management is about helping our clients to improve their quality of life.

Case Managers act as a bridge and help clients connect with needed services including physicians who specialize in mental illness, employers and housing that helps create a path to recovery. 

Salus offers a range of intensive case management services in English, French, and American Sign Language (ASL) and Langue des signes québécoise (LSQ).

 

WHAT DO SALUS CASE MANAGERS DO?

Salus Case Managers provide individualized, flexible, and client directed support to individuals living with a severe and persistent mental illness. The focus is upon client strengths, interests and abilities and helping them build a foundation for taking ownership of their own recovery process. Case Managers meet with clients in their own environments and follow the clients when they change housing.

 

HOW IS CASE MANAGEMENT DELIVERED?

Salus Case Managers and their clients work together to:

  • Assess client strengths and needs in all life areas,
  • Identify goals for change, and
  • Develop a service plan.

With a service plan in place, the Salus Case Manager then assists the client in meeting individual goals by providing supportive counseling, practical assistance, coordination and linking of services, as well as advocacy.

Salus is a partner agency in the Mental Health Community Support Services coordinated by the Canadian Mental Health Association which operates client intake and assessment.
